Customer Work Authorization Form

This is an agreement between you and Jay's Computers and Repair LLC, d.b.a. Jay's Computers and Repair, to perform work including, but not limited to, computer and electronic repair, consulting, training, design, installation, and other related tasks.

By entering your name below and submitting this form you are agreeing to allow Jay's Computers and Repair LLC and its authorized agents to perform the work described in the estimate issued to you and you agree to the terms set within the estimate and this authorization form.

You acknowledge that, within the scope of performing computer repair tasks, Jay's Computers and Repair LLC may access files and folders on the equipment to be repaired and that any such access will be incidental to the work performed. Jay's Computers and Repair LLC will not disclose any information found during the course of repairs unless such disclosure is required as provided for by local, state, and federal law. Jay's Computers and Repair LLC will be bound to maintain customer privacy at all costs and any backups, copies, or other representations of customer data will be permanently destroyed after fourteen (14) days of completion of repair unless agreed upon in writing by both parties. Customer data on hard drives and other long-term storage media tendered to Jay's Computers and Repair LLC will be destroyed if no longer needed after fourteen (14) days unless otherwise agreed upon in writing by both parties.

Jay's Computers and Repair LLC will not be held responsible for loss of data. While every effort will be made to maintain data integrity, including backup copies and other safety measures, it is ultimately the customer's responsibility to ensure that important data is backed up before the equipment to be repaired is tendered.

I understand that my data may be lost and that I have a suitable backup.

Equipment that is repaired must be picked up within 7 days unless prior arrangements have been made. Equipment left longer than 14 days will become the property of Jay's Computers and Repair LLC and will be disposed of, recycled, reused, resold, or repurposed, and any claim to said property will be forfeit.

Invoices are due upon completion of work unless otherwise agreed upon by both parties in writing. Invoices not paid after 15 days will be subject to a 3% fee. Invoices remaining unpaid after 30 days will incur a 6% late fee. After 45 days collection action will be taken.

All work performed by Jays Computers and Repair LLC is guaranteed for a period of 30 days. Parts may have longer warranty terms by manufacturer - inquiries are always welcome on warranty information for parts used. Defective parts will be replaced or refurbished at Jay's Computers and Repair LLC's sole discretion.
